  1. Set a dry medium skillet over medium heat. Add the 1/2 cup masa harina and toast, stirring constantly with a wooden spoon, for 3 to 4 minutes, until it smells like warm tortillas and turns one shade darker. Do not let it brown or scorch; pull it off the heat immediately if it starts to smoke, and slide it into a bowl to stop the cooking.
  2. In a bowl, whisk the toasted masa harina with the 2 cups cold water and the salt until completely smooth, about 1 minute. Cold water is important — hot water will seize the masa into lumps. Let it sit 5 minutes, then whisk again and pour through a fine-mesh sieve into a measuring cup, pressing any stubborn bits through with a spatula. Discard whatever grit stays behind.
  3. In a heavy 3- to 4-quart saucepan, combine the milk, chopped piloncillo, cinnamon stick and orange zest strip. Scrape the vanilla bean seeds into the pot and add the pod too (if using extract, hold it back until the end). Warm over medium-low heat, stirring, for 8 to 10 minutes, until the piloncillo has fully dissolved and the milk steams but does not boil. Keep it below a simmer so it doesn't scorch on the bottom.
  4. Pour the strained masa slurry into the hot milk in a slow stream while whisking constantly. Raise the heat to medium and bring the atole to a bare simmer — you want lazy bubbles breaking the surface, not a rolling boil.
  5. Cook at that bare simmer for 10 to 12 minutes, whisking every 30 seconds and scraping the bottom corners of the pan, until the atole thickens enough to coat the back of a spoon and leave a clear line when you drag a finger through it. The raw-starch taste should be gone; if it still tastes chalky, give it 3 more minutes.
  6. Off the heat, fish out the cinnamon stick, orange zest and vanilla pod. Stir in the evaporated milk if using, plus the vanilla extract if you didn't use a bean. Taste and add more piloncillo or a pinch of salt as needed — atole should be gently sweet, not dessert-sweet.
  7. Whisk hard for 15 seconds to smooth and lightly froth the surface, then pour into warm mugs. Serve immediately while it is thick and steaming; it is traditionally drunk, not spooned, so it should flow slowly but still pour.
