  1. Grate all three cheeses on the smallest rasp-style holes you have and toss them together in a bowl so they're evenly mixed. Fine, fluffy shreds melt into a sauce; thick pre-shredded strands and anti-caking starch will clump and string. Let the cheese sit at room temperature while the water heats.
  2. Bring 4 quarts of water to a boil in a large pot with 2 tablespoons kosher salt — less water than usual, because you want it aggressively starchy. Add the fettuccine and cook, stirring often, until just shy of al dente: about 1 minute less than the package says (roughly 8–9 minutes for dried, 2–3 minutes for fresh).
  3. Meanwhile, set a large skillet or sauté pan over low heat. Add 4 tablespoons of the cold butter and the optional heavy cream, and swirl just until the butter melts into a cloudy pool — do not let it bubble hard or brown. Kill the heat.
  4. When the pasta is 1 minute from done, dip out 2 cups of the cooking water and reserve it. Drain the pasta briefly (do not rinse) and add it to the skillet along with 3/4 cup of the hot pasta water.
  5. Turn the heat to medium-low and toss the pasta constantly with tongs for 60–90 seconds. The water and starch should look creamy and start clinging to the noodles. This starchy base is what will hold the cheese; skipping it is the number one reason alfredo breaks.
  6. Pull the pan completely off the heat and wait 15 seconds so it drops below a simmer (cheese seizes and turns grainy above about 175°F). Add the remaining 2 tablespoons cold butter and toss until melted, then add the cheese in three additions, tossing hard after each and splashing in 2–3 tablespoons pasta water with every handful.
  7. Keep tossing and swirling the pan for 1–2 minutes until the sauce is glossy, thick and coats each strand like a light gravy. If it looks tight or clumpy, add pasta water a tablespoon at a time and keep moving; if it looks thin, return the pan to low heat for 20–30 seconds while tossing, then remove again.
  8. Season with the black pepper, the pinch of nutmeg if using, and flaky salt only if needed — the pecorino brings a lot. Serve immediately in warmed bowls, showered with more grated parmesan, extra pepper, and parsley if you like. Alfredo waits for no one; it firms up within a few minutes.
